Student Record Request

To request a copy of your transcript for the time you attended Calexico Unified School District, fill out this form and return to the Instructional Service office.

Please include the following in your transcript request:
    * The name you used while attending school
    * Your date of birth
    * Birthplace
    * Parents’ name (s)

Please indicate if your transcript needs to be:
    * Official – for use with colleges, employment, immigration purposes
    * Unofficial – for personal use
